The FORM 137 (F-137) refers to the scholastic record of student
from Grade School and High School Department
Student may request their Transcript of Records for the following
purposes:
- Requested by the school currently enrolled
- For Studying Abroad
- For Entrance Examination in other
Universities
- For Employment (Local and Abroad)
Follow the below procedures to request for the Transcript of Records and
visit the Registrar’s Office
- Fill out the Request Form and submit to
the Registrar’s Office.
- Secure Return Slip with the specified date
of issuance from the Registrar’s Office.
- Pay for the requested FORM137 to the
Cashier’s Office.

- Claim the FORM137 at the Registrar’s Office
on the scheduled due date.
Note:
If requested by the representative, present the following:
- Authorization letter
from the requesting party
- A valid Identification from the
representative and requesting party
